Could Acetyl L-Carnitine (OTC) supplements be harmful? A cursory, and overtired, review of https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2905823/ seems to suggest that a low ratio of Acetyl L-Carnitine, or “AC” to Free Carnitine (FC) is healthier:

On the other hand a study, https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4255476/, indicates that increased carnitine intake (in the form of red meats) increased atherosclerosis via the gut microbe metabolite TMA (hepatically converted to TMAO), although the aforementioned article suggests a direct cardioprotective metabolic mechanism of free carnitine. Interesting, and time-consuming stuff!
